Job Summary
The Social Worker will provide primary support for clients (and their families);will coordinate accessing the supports and services necessary to support the adult and their family to achieve the goals of their plan.
Primary Duties and Responsibilities
1. Provide counseling and case management services for at-risk, homeless and formerly homeless individuals entering or at risk of losing housing;
2. Monitor consumer progress and serve as case manager for program participants;
3. Provide direct service to consumers around issues related to independent living and barriers to maintaining housing
4. Assess consumer barriers and develop a service plan with housing goals, and where applicable refer to mental health services, develop vocational goals and implement a plan of action with the consumer;
5. Enroll consumers into computerized case management system (CICERO) and maintain case management records in CICERO in a timely and accurate fashion;
6. Maintain consistent contact with consumers in assigned caseload, monitor progress refer to designated agencies and individuals according to case management goals, coordinate referrals as necessary;
7. Facilitate consumer workshops and group activities;
8. Participate in staff, inter-agency or other program coordinating meetings; in-house training and staff development meetings, case conference, and staff meetings, and represent the program at specified interagency gatherings; and
9. Perform other duties as assigned.
10. Timely completion of all required paperwork regarding clients and services provided.
Skills:
Minimum Qualifications
1. Background in mental health; masters’ level and license eligible, or equivalent experience. MSW preferred.
2. Experience providing clinical services to children and families.
3. Bilingual in Spanish.
4. Interest in and sensitivity to people from a multiplicity of backgrounds (ethnic, religious, social class, sexual orientation, etc.), with core awareness and sensitivity to the cultures and needs of the Latino communities.
5. Current drivers license and auto insurance
